결론 및 제언
가정 간호 대상자의 기구 사용에 대한 현황을 파악하고자 1998년 8월 1일부터 12월 30일
까지 5 개월간 서울시 간호사회에 등록된 가정간호사 7명과 그들이 담당하고 있는 간호대상
자 전수인 88명을 조사하였다.
자료분석은 SPSS PC를 이용하여 평균, 백분율을 구하였으며 결과는 다음과 같다.
1, 가정간호대상자 중 여자가 55.7%(49명)이었으며 노인환자가 51.7%(45명)이었다. 질병별
로는 신경계 환자가 75.0%(66명)였고 대부분(78.4%)이 서울거주자였다. 가정간호사는 최소
5년 이상의 높은 경력을 소지하였다.
2. 가정간호 대상자의 기구 소지는 정체도뇨관(61.4%), 비위관(59.1%), 기관절개관(51.1%)
순으로 나타났으며 첨단 의료장비인 인공호흡기 착용자도 5.7%로 나타났다. 기구를 부착하
고 있는 대상자는 전체의 93.2%(82명)이었다.
3. 기구 사용시의 기술적인 어려움은 인공호흡기가 60.0%로 가장 많았고 비위관 3.8%, 기
관절 개관 2.2%로 기술적 어려움은 거의 없는 것으로 나타났다.
4. 기구 구입에 있어서는 대상자가 입원했던 병원이나 대상자 자신이 구입하는 경우가 대
부분인 것으로 나타났다. 그러나 가정간호사가 의료기상사를 통하여 구입해주는 경우도 상
당부분 있어서 상처 배액관의 경우 2사례 모두, 그리고 정체도뇨관은 48.1%로 나타났다.
5. 기구 사용시 합병증이 발생한 경우는 평균 9.9%였으며 상처 배액관의 경우 50%, 정맥
주사는 22.2%로 주로 침습성 절차의 경우인 것으로 나타났다. 또한 이의 해결방법으로는 대
상자가 입원했던 병원에 의뢰하는 경우가 74.7%였으며 보건소 의사에게 의뢰하는 경우는
13.24%였다.
6. 기구 사용에 대한 교육은 매달 개최되는 가정 간호사 자체 세미나를 통한 경우가
47.65%로 가장 많았고, 학회나 가정간호사회 등에서 교육받는 경우는 37.5%였다.
7. 기구 소독의 경우에는 정체도뇨관, 비위관, 인공루, 상처배액관은 주로 1회용을 사용하
는 것으로 나타났으며 기관절개관은 가정간호사의 기호에 따라 물로 끓이거나 병원에 의뢰
하는등 다양한 방법을 사용하였다.
8. 기구사용시의 소모품은 매우 다양한 것으로 나타났다. 즉, 정맥주사의 경우 소독용 솜
을 알코올, 붕산수, 베타딘 등으로 간호사에 따라 각기 다른 약제를 사용하였다.
이상의 결과를 종합해 볼 때 가정간호 대상자들의 90%이상이 기구를 사용하였으나 기구
의 구입이나 관리 등에 대한 일관된 정책이 마련되어 있지 않아서 간호 대상자에 대한 안전
에 위협을 줄 수 있으며 이에 따른 재정적 부담을 가중시키고 있는 것을 알 수 있었다. 그
러므로 가정간호사업의 발전과 활성화를 위한 의료기기 및 장비의 효율적 공급 및 지원 방
안에 대한 연구가 계속되어야 할 것이다.

This study was conducted to investigate the use of medical equipment in patients
receiving home care service. The subjects of this study were 88 patients cared by seven
home care nursed who were registered in the Seoul Nurses Association. Data was
collected from Aug. 1, 1998 to Dec. 30, 1998.
The findings are as follows.
1. The sample was found to be 55.7% female : 51.7% over 65 years old. 75% with
neurologic disease including CVA, brain tumor, ICH, Parkinsonism & Spinal stenosis and
78.4% living in Seoul. The Clinical experience of the home care nurses was greater than
five years.
2. Medical equipment which the patients port sensed were folly catheters(61.4%),
L-tubes(59.1%) and tracheostomy tubes(51.1%).
3. Technical difficulties in use of medical equipment were related to home care
ventilators(60.0%), L-tubes(3.8%) and tracheostomy tubes (2.2%)
4. Most of the medical equipment were obtained from the hospital where they had
been admitted previously or from medical equipment companies.
5. Complications from the use of this equipment were infection through invasive
techniques Including wound drainage tubes(50%), and Ⅳ injections(22.2%), The
complications were resolved through referral to the doctor of the hospital where they
were previously admitted or through community health centers.
6. Most of the equipment was disposable, and equipment was disinfected by using
various methods including boiling and soaking in antiseptic solutions.
These findings suggest that consistant policy on the management of medical
equipment is necessary for the safety of home care patients.
